Payment Options
How do I pay for this?
At the Artz Center for Developmental Health & Audiology we wish to provide you with thorough information regarding your insurance benefits, fees, and services. This includes:
Insurance benefit checks: Our client service coordinators will contact your insurance company to collect information about your available benefits, applicable deductible amount, co-pay and/or co-insurance, number of visits covered, and maximum dollar amount your insurance will cover.
Changes in insurance: Should you have a change in your insurance it is important that you let us know right away. Be sure to contact us before your next visit to allow us time to check your new benefits. You will also be asked to present your new card at check-in so that we can make a copy for our records.
Self-pay : Clients who pay in full at the time of service will receive a 15% discount off the cost of that day’s visit.
Co-pays and Co-insurance: Payment for co-pays/co-insurance is due in full at the time of service. Payments not received at the time of service will be assessed a $20 fee.
Deductible: If your deductible amount has not been met you will be asked to pay in full at the time of service until your deductible has been satisfied with your insurance company.
Payment plans: At times there may be a need to set up a payment plan. Our billing department will work with you to set up a reasonable payment plan.
Financial hardship: If you are experiencing a financial hardship, you may be eligible for scholarship funds. Please contact our scholarship coordinator by calling the front desk at 503-228-6479.
Finance charge: Clients with account balances will receive a billing statement every 30 days. Balances that are unpaid after 60 days will accrue a finance charge of 12% annually.
